SAN J O A Q U I N Environmental Health Department Time In: 2:38 pm Time Out: 3:15 pm COMPLAINT INSPECTION REPORT Name of Facility: WALMART#1789 Date: 12/07/2020 Address: 1601 S LOWER SACRAMENTO RD, LODI 95242 Owner/Operator: WAL-MART Telephone: (209) 368-6696 Program Element: 1600 - FOOD PROGRAM Complaint#: C00053083 Responsible Party: DESCRIPTION OF COMPLAINT ALLEGATIONS The San Joaquin County Environmental Health Department received the following complaint: COMPLAINANT ALLEGES THAT HE OBSERVED A PACKAGE OF HAMBURGER MEAT THAT CONTAINED BLOOD. COMPLAINANT ALLEGES HE ALSO OBSERVED BLOOD ON THE OTHER PACKAGES OF THE HAMBURGER MEAT. COMPLAINANT ALSO OBSERVED THE THREE PAPER TOWEL DISPENSERS NEARBY WERE EMPTY. VIOLATIONS AND CORRECTIVE ACTIONS Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health and have the potential to cause foodborne illness.All major violations must be corrected immediately. Non-compliance may warrant immediate closure of the food facility. OVERALL INSPECTION NOTES AND COMMENTS FOOD ITEM --LOCATION --TEMP° F--COMMENTS No Temperature Data Collected NOTES I went to the facility and spoke to the manager who was not aware of the complaint prior to my investigation. Per manager, all meat comes to the facility pre-packaged and stored in the meat walk-in and then in the meat wall refrigerator on the sales floor. I observed the meat wall refrigerator on the sales floor. I observed packages of raw meat and raw hamburger meat. I did not observe"Blood on other packages of hamburger meat.." I did not observe blood on the exterior of meat packaging. I also observed sanitizer and paper towel stands that are set out approximately six feet apart in front of refrigeration cases. There were paper towels in one dispenser, another dispenser had paper towels but the paper towels were stuck, and one dispenser was out of paper towels. Paper towels were supplied to the empty dispenser and the dispenser that was not dispensing towels was adjusted so the towels dispensed properly. The dispensers were addressed during inspection. The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and Safety Code. If a reinspection is required,fees will be assessed at the current hourly rate.
