Complaint Investigation Summary Report#: 5105 <br /> COMPLAINT ID: COOYFE7IR Site Location: 8727 ST RT 99 W FRONTAGE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 06/12/2020 Received page at 1:36 am from SJGH. Called 209-468-6100 and phone was BAKER <br /> busy. Called 209-468-6000 and spoke with Operator. She said that Henry Roter <br /> of Frank Alegre Trucking reported a 30 gallon diesel spill from a roll over. Spill <br /> occurred on the southbound 99 Frontage Road at the French Camp exit. <br /> 1:48 am-Called Henry Roter at 209-810-5290 and left a message. <br /> 1:58am-Left for site. <br /> 2:37 am-Arrived on site. Met with Toby (Shop Foreman) for Frank C. Alegre <br /> Trucking. He said that Safety Kleen would be handling the clean-up of the diesel. <br /> Trailer was carrying flour. No flour was released as a result of accident. Truck <br /> was still in the process of being turned over from it's side and towed away. <br /> Approximately 30 gallons of diesel was released onto the soil. Visible spill area <br /> covered approximately 8x8 feet. No waterways or storm drains were impacted by <br /> the release. <br /> Created complaint, emergency response record and prop 65. Verified GIS in EC. <br /> Wrote Notice to Abate. Emailed notice to abate to Henry Roter(Safety Manager) <br /> at henry@alegretrucking.com. Called Pat with Safety Kleen at 209-480-1071 to <br /> get an estimated time of arrival. Pat said that Norm Sanchez (Clean <br /> Harbors-408-472-7710)would be out doing the clean-up and would not arrive for a <br /> couple hours. <br /> 3:30 am-Left site. <br /> 3:47 am-Received a call from Norm Sanchez. He said that he was on site and <br /> there still was a lot of work for the tow truck drivers to do. He said Clean Harbors <br /> has a commitment to do another clean-up that morning and would be done around <br /> 12-1 pm and would then start this clean-up. Norm Sanchez said he would let me <br /> know when they were headed to the site to start the clean-up. <br /> 4:20 am-Arrived back. Completed photo log. <br /> 1:36am-4:36am <br /> 06/15/2020 Put copy of notice to abate on shelf to be mailed to property owner regular mail. BAKER <br /> Gave Prop 65 to Elena Manzo (Lead Sr. REHS) for review. <br /> Completed photo log. <br /> 06/15/2020 Mailed Waste Discharge/Spill Response Notice to Abate dated 06/12/20 by MARTINEZ <br /> regular mail to: <br /> SAN JOAQUIN COUNTY, <br /> 5000 S AIRPORT WAY STE 202, <br /> STOCKTON CA 95206. (Also mailed Prop 65#20-072 in the same envelope). <br /> 07/14/2020 Emailed Henry Roter(Safety Director) at henry@alegretrucking.com for a copy of BAKER <br /> the disposal record. <br /> 07/22/2020 Received update on 7/17/2020 from Henry Roter that the waste profiles have been BAKER <br /> received and a pick up date for the drums has been scheduled. <br /> 5105.rpt Page 2 of 3 <br />
