Time In: 11:15 am <br /> r <br /> Time Out: 11:45 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: JACK IN THE BOX#493 Date: 02/18/2021 <br /> Address: 419 W LODI AVE, LODI 95240 <br /> Owner/Operator: ANIL YADAV Telephone: (209)369-4590 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00053450 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT PURCHASED AND CONSUMED TWO TACOS FROM THIS FACILITY AND ALLEGES THAT ALMOST <br /> IMMEDIATELY THEY EXPERIENCED A REACTION AND WAS STILL ILL WHEN REPORTED. ONSET SYMPTOMS: <br /> WITHIN 48-72 HOURS COMPLAINANT EXPERIENCED VOMITING, FATIGUE, NAUSEA, FEVER,AND ABDOMINAL <br /> PAIN. COMPLAINANT DID NOT SEEK MEDICAL ATTENTION. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke with the manager who was not aware of the complaint prior to this investigation. <br /> The tacos come to the facility premade(taco shell and filling)and tacos are in pre wrapped packages and are frozen <br /> The frozen packages of tacos are kept in the walk-in freezer until brought to the walk-in refrigerator(41 F)to defrost. <br /> Tacos are taco shel filling, cheese, lettuce and hot sauce. <br /> Cheese and lettuce come to the facility and are stored in the walk-in until prepared. <br /> Lettuce is washed and cut on a cleaned and sanitized prep table and shredded lettuce is kept in walk-in. <br /> I observed shredded lettuce that is stored properly and covered in walk-in. <br /> Cheese is cut on sanitized prep table. I observed cut cheese covered in the walk-in. <br /> Tacos, cheese, and lettuce are stored in the reach-in next to the deep fryer(40F). <br /> Cheese and shredded lettuce are at the taco station and have a 4 hour hold time. <br /> Tacos are prepared when ordered.) observed a taco prepared.Taco temperature is at 180F. <br /> Per manager there have been no ill employees for the last two weeks. <br /> FA0001047 C00053450 SCO04 02/18/2021 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 2 Food Program Complaint Inspection Report <br /> 1868 E.
