Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO0052084 Site Location: NEAR 942 STOCKTON AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 06/17/2020 Received pager notification from Yvonne Moreno, Operations. Gave me the GAITAN <br /> contact information for Nathan Christman, Environmental Specialist for PGE. <br /> Reported that approximately 1 gallon of hydraulic oil was released on to soil. <br /> Located near 942 S. Stockton Ripon, CA <br /> Contacted Nathan he stated that hydraulic oil was released due to a faulty <br /> mechanical error on the backhoe on site. Nathan stated that released has been <br /> contained and stopped. He stated that PGE crew would arrive with a 55-gallon <br /> drum and clean-up hydraulic release. We exchanged emails and gave me the <br /> contact information of the foreman on site, John Jackson. <br /> Contacted Elena Manzo ILead Senior REHS1 discussed complaint. After <br /> discussion, I contacted Nathan and let him know that I will be going to verify clean <br /> up. <br /> Entered Diamond Pet Food, spill is located on the back of the facility. Met with <br /> John Jackson, Foreman. He stated that backhoe hydraulic line tore and slowly <br /> released the oil. He stated that soil had been shoveled up; I could not observe any <br /> staining on the soil. I observed that the hydraulic oil was still actively leaking from <br /> the backhoe and was being accumulated in a black oil pan. I observed the <br /> following waste on site: <br /> -Two 5-gallon bags with soil contaminated with hydraulic oil. <br /> -One 5-gallon bucket with rags contaminated with hydraulic oil. <br /> -One oil pan with accumulating hydraulic oil. <br /> John stated that he does not know when PGE mechanic will be arriving to fix the <br /> backhoe and when additional clean up will be occurring. John stated that if clean <br /> up and maintenance is not performed today, he will caution tape off the leaking <br /> backhoe and waste generated. <br /> Contacted Nathan notified him that hydraulic oil is still actively leaking. Nathan <br /> stated that he was unaware that hydraulic oil was actively leaking. I asked if he <br /> knew when the PGE mechanic was arriving, he stated that he does not have an <br /> ETA. Contacted Elena and updated her on spill. After discussion, I generated a <br /> notice to abate. Sent notice to abate to Nathan and Alex Steele, Environmental <br /> Scientist for PGE. <br /> Discussed complaint with John Alaniz (RENS). Sent John copy of NTA, photos, <br /> and 5104 report. <br /> 06/18/2020 Downloaded the photos and created the photo log. Emailed to area inspectior. ALANIZ <br /> 06/18/2020 Generated photo log. Discussed complaint with John Alaniz (REHS). GAITAN <br /> 06/19/2020 Generated prop 65. Gave to Elena Manzo (Lead Senior REHS) for review. Mailed GAITAN <br /> to PGE and property owner. <br /> 09/30/2020 File review. Put ER file together. Sent email to Nathan Christman, Senior GAITAN <br /> Environmental Field Specialist for PGE. Requested disposal receipt for waste <br /> generated from clean up. <br /> Contacted Nathan. Discussed disposal receipt, he stated that waste was picked <br /> on September 11 th and he is waiting for the designated copy of the uniform <br /> hazardous waste manifest. He provided the following temporary EPA ID number: <br /> CAC003081826 Searched DTSC site to find manifest. Could not locate it. <br /> 10/05/2020 Received email from Nathan Christman, environmental field scientist. Provided GAITAN <br /> generator copy of uniform hazardous waste manifest for waste generated from <br /> clean-up. UHWM: (016321169JJK).Attempted to locate designated facility copy <br /> on DTSC website and RCRA online website. Could not find a copy. <br /> 10/07/2020 Reviewed narrative.
