Complaint Investigation Summary Report#: 5105 <br /> COMPLAINT ID: COMXHD2JB Site Location: 1640 SPRING COURT <br /> Activity Summary <br /> Activity Date Recorded by <br /> 04/15/2020 8:32 PM Received page from SJGH 209-468-6000. 1 called and spoke with MURO <br /> Yvonne (Dispatcher at SJGH).She reported that Larry Vickers (Chief at Tracy Fire <br /> Department) called in a half gallon fuel spill at 1640 Spring Court in Tracy. Larry <br /> Vickers' callback number is: 209-601-3884. <br /> 8:36 PM Called Larry Vickers who provided the following information: <br /> - Responsible party,Antonio Pelayo, 408-464-0478 performed an oil change on <br /> his vehicle then washed his car. Tracy Fire personnel believe that Antonio Pelayo <br /> may have been trying to clean oil with the fuel. Though Antonio told fire <br /> department personnel that he did not use fuel to clean oil. <br /> - It was alleged that approximately 1/2-gallon of fuel was released onto gutter and <br /> storm drain. Larry reported that Tracy firefighters observed the gutter to be wet, <br /> but with no puddling. They observed the storm drain lid to be dry. <br /> -Tracy Fire applied absorbent over the spilled gasoline on the gutter. They were <br /> unable to open the strom drain lid. <br /> -There was a smell of fuel vapors in the spill area. <br /> -They called EHD to advise on how to proceed. <br /> -Tracy Fire estimates only one bucket(5-gallon capacity) of total <br /> gasoline-contaminated hazardous waste will be generated. <br /> Consulted Program Coordinator, Muniappa Naidu. He advised that Tracy Fire <br /> Department contact City of Tracy Public Works since the storm drain was <br /> allegedly impacted. <br /> 8:50 PM Called Larry Vickers and asked that they contact City of Tracy Public <br /> Works for cleanup verification. He told me they have the City of Tracy Public <br /> Works after-hours standby phone number and would call them. He requested the <br /> complaint number. I told him I would email it once I generate the complaint. Larry <br /> provided his email address: larry.vickers@cityoftracy.org <br /> Generated complaint. <br /> Generated Notice to Abate. <br /> 9:07 PM and 9:10 PM Called Responsible party Antonio Pelayo (408-464-0478) <br /> two times, but he did not pick up call and mailbox was full. <br /> Emailed complaint number to Larry Vickers, larry.vickers@cityoftracy.org, and <br /> requested additional contact information for Antonio Pelayo. <br /> End <br /> 04/16/2020 Received reply from John Thompson, Tracy Fire Department Captain assigned to MURO <br /> the 1640 Spring Ct call. He did not have additional contact information for Antonio <br /> Pelayo. <br /> Emailed Notice to Abate to Lead Senior in the office today, Haza Saeed so that he <br /> can assist with printing and placing in the outgoing mail box. <br /> 5105.rpt Page 2 of 6 <br />
