Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COMXP9XLQ Site Location: WB SR 26 , 1/2 MILE EAST OF WALL RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 11/13/2020 4:12-5:00 PM ALANIZ <br /> Received CalOES spill report 20-6343. A PG&E truck got into an accident and <br /> released approximately 13 gallons of hydraulic oil. Called CHP dispatch and got <br /> connected with Sergeant Barbara Butler. Sergeant Butler gave her phone to PG&E <br /> supervisor, Sean Giangregorio <SPGc@pge.com>,209-337-8851. 1 informed him <br /> that I was going to be leaving to the site and that I would be writing a notice to <br /> abate. He said that PSC (environmental company)was on their way to clean up the <br /> spill. <br /> Left for the site. On the way to the site I was paged by the EHD. Alex Steele, <br /> PG&E, was making a notification to the department. Called him at 209-337-8902. <br /> He informed me that he was going to be the contact point for the cleanup. <br /> 11/14/2020 1:00-4:00 P.M. ALANIZ <br /> Met with Dave Larsen, PG&E, and Kevin Bottin, PG&E. The cleanup was <br /> complete. Five and a half 55 gallon drums of oily debris was generated. The <br /> excavated area was approximately 8ftx3ft. The depth varied from approximately 1-4 <br /> feet. Dave said the waste was going to go to the 4040 West Ln PG&E yard for <br /> disposal as non-hazardous waste. The waste determination was made through <br /> generator knowledge. <br /> Called Alex Steele, PG&E, 209-337-8902, and left a message regarding the <br /> disposal of the waste. Sent an email to him at<R2Sv@pge.com> asking how the <br /> generator knowledge determination was made. <br /> Sent another email to Alex with the following people cc'd: <br /> Loren Shelton at<loren.shelton@dot.ca.gov> <br /> Juan Armendarez at<juan.armendarez@dot.ca.gov> <br /> Robert Shanks at<Robert.Shan ks@dot.ca.gov> <br /> Sergeant Barbara Butler at<bbutler@chp.ca.gov> <br /> The email let Alex know that the spill was cleaned up and how much waste was <br /> generated. <br /> 11/14/2020 11:12-11:18 ALANIZ <br /> Called Dave Larsen, PG&E. He said he would update me on the spill at 11:00 A.M. <br /> Called Dave to get an update on the spill. No answer and the mail box was full. <br /> Called Dave to get an update on the spill. Dave said the cleanup would be <br /> completed at 2:00 P.M. <br /> 11/16/2020 Discussed making a hazardous waste determination with Alex Steele. He said he ALANIZ <br /> would send me the proper documentation showing how the waste was disposed of <br /> and how the determination was made. <br /> Completed the photo logs from 11/13 and 11/14 <br /> Completed prop 65 <br /> Completed complaint information for spill location and er record <br /> 5105.rpt Page 2 of 3 <br />
