Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COPMS2TWF Site Location: MANHOLE IN FRONT OF 4058 CASTELLINA WAY <br /> Activity Summary <br /> Activity Date Recorded by <br /> 12/26/2020 5:18 PM-7:00 PM ALANIZ <br /> Received a page from the general hospital letting me know that Erin <br /> (916-862-3754)with Public Health called to speak with me. Received spill report <br /> 20-7116 via email on the ER phone. An unknown amount of sewage released <br /> from a manhole in front of 4058 Castellina Way. <br /> Called Erin confirmed that I received the spill report. <br /> Called Casey Wichert, Oakwood Lake Water District, (209-483-5525), who <br /> reported the spill. The following information was gathered (over several phone <br /> calls between me, Muniappa Naidu, Haza, and him): <br /> -Only six houses tie into the sewer line. As a result, Casey did not think a lot of <br /> sewage went into the storm drain. <br /> -Casey inspected further down the line and did not see any visible sewage in the <br /> storm drain. The storm drain was blocked off to prevent any further <br /> contamination. <br /> -Casey said that the storm drain was located in an unincorporated part of the <br /> county. <br /> -Hydro-X was hired to clean the spill. A vacuum truck was brought to unclog the <br /> sewage drain. After the drain was unclogged, the affected areas (included the <br /> storm drain)were washed with water. The waste water was sucked up and put <br /> back into the sewage line for disposal. <br /> -Casey stated that he did not receive any reports of affected homes. The sewer <br /> lines come out from the front of the house. No homeowner reported sewage <br /> problems inside the house. <br /> -The spill was cleaned up at approximately 6:55 PM. Casey sent me photos of the <br /> final clean up. <br /> During the time speaking with Casey: <br /> -1 looked up the property owner information in parcel viewer to make sure it was <br /> unincorporated. <br /> -1 called Robert McClellon, program coordinator, and informed him what <br /> happened. He asked that I email him the spill report. <br /> -Received a page from Johnathan with San Joaquin County Public Health. He <br /> wanted to confirm that I received the spill report information. He said that <br /> Oakwood Lake was not a public drinking water system and he did not have any <br /> concerns about the incident. <br /> -Called the storm water inspector number in the ER phone list and it did not work. <br /> -Called the 24 hour San Joaquin County storm water number and the message <br /> directed me to call the sheriff. <br /> -Called the Public Works after hours phone number and spoke with Chris Zarate. <br /> He said he would be the one to take the call. Emailed him the spill report and he <br /> said he would give it to the right people. <br /> -Received a page from the hospital asking me to call Josh with CalOES, <br /> 916-845-8911. He wanted to confirm that I received the spill report. <br /> 12/28/2020 -Emailed the spill report to Michael Kith, program coordinator. ALANIZ <br /> -Created the complaint and completed the narrative <br /> -Created a photo log <br /> 01/04/2021 Gathered the complaint documents and abated the complaint.
