Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COZGGOIHY Site Location: 1000 E ROTH RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 02/09/2020 11:30 AM - Received Cal OES spill report#20-0788 and an update to this report. FLORIDO <br /> Called and spoke Brad Douglass of Union Pacific Railroad (UPRR) and was <br /> referred to Mike Algots, UPRR Haz Mat Manager(816-308-8913). Called Mike <br /> who verified less than 10 gallons of diesel fuel, initially reported as transmission <br /> fluid, released from a refrigeration container from it's damaged motor. The release <br /> impacted concrete only, was stopped, the leaking container was transferred to the <br /> leaker pad, and absorbent was applied to the spill on the Ieaker pad by UPRR <br /> personnel. Per Mike the responsible party is UPRR and the waste generated from <br /> the spill clean-up will be kept on site until the facility's next waste pick-up. A <br /> sample of the waste will be taken for analysis by the clean-up contractor to <br /> determine if it is hazardous. Patriot Environmental is en route to the spill location <br /> to perform the clean up. <br /> Called and updated Muniappa Naidu, CUPA Program Coordinator, about the spill. <br /> Called and left a voice message for Brandon Eck, Patriot Environmental <br /> Supervisor(916-767-9598), who is en route to conduct the clean-up. Called and <br /> briefly spoke to Derek Smith of Patriot Environmental (916-347-8724), when I <br /> received a call back from Brandon. Spoke to Brandon and let him know to contact <br /> me when he is about 45 minutes from completing the clean-up. Brandon agreed. <br /> Called Derek back and let him know that I informed Brandon to contact me when <br /> he close to completing the clean-up. Derek understood. <br /> Text messaged Brandon from the ER cell phone requesting he send me photos of <br /> the spill areas and an estimated time for when he would be finished with the <br /> clean-up. <br /> 12:00 PM - END <br /> 12:42 PM - Received a text message from Brandon with photos of the spill areas <br /> and stated his crew would be on site within 30 minutes to start the clean-up and it <br /> would take them at most 1.5 hours to complete. Brandon recommended I leave <br /> my house around 1:45pm so that I arrive on site as they are completing the <br /> clean-up. <br /> Generated complaint and began generating the Emergency Response Record <br /> form and Prop 65. <br /> 2:18 PM -Arrived on site and checked in at the UPRR front office. Met Brandon at <br /> the Ieaker pad. Observed absorbent staining on the concrete leak pad beneath <br /> the affected refrigeration container. The spill was stopped and the leak pad <br /> appeared clean. Took photos of spill area. Per Brandon, the diesel spill on the leak <br /> pad appeared cleaned up before he arrived on site, and he did not know where the <br /> used absorbent was placed. After discussions with Mike Algots, and Brandon, it <br /> was determined that the absorbent used-a 25 Ib. bag-on the leak pad spill was <br /> blown away by the heavy winds before Brandon arrived on-site. <br /> Brandon escorted me to the initial spill location near the train tracks. Observed <br /> absorbent staining on the concrete next to the track. The spill clean-up was <br /> complete in this area. Took photos of spill area and of the 55-gallon drum of waste <br /> generated. Brandon stated he found no evidence of the spill in any other areas. <br /> Brandon stated that he found two smaller diesel spills on concrete in the yard that <br /> appeared to be unrelated to the original diesel spill. <br /> Generated the Notice To Abate as Brandon cleaned up these two spills and <br /> placed the used absorbent into the 55-gallon drum of used absorbent from the <br /> original spill. Brandon took a sample of the waste and labeled the drum as <br /> 5105.rpt Page 2 of 4 <br />
