This information was <br /> required to be submitted by 7/24/2020. This information has not been received resulting in a non-compliant status <br /> for this facility. <br /> REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include <br /> all of the recommendations made in the report. The operator shall submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted and all corrections implemented within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class II. <br /> 206 CCR 2712(i), 2634(e)Failure to have a current UST Response Plan available on site. <br /> OBSERVATION:An approved copy of the most current response plan was not available for inspection.The <br /> response plan on site was dated 3/19/2009 but the most current response plan in CERS is dated 6/4/2019. <br /> REGULATION GUIDANCE:A copy of this plan shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the <br /> 5/27/2020 and 6/24/2020 designated operator inspection reports and failed to check that they were responded to <br /> appropriately. The missing alarms include: L3 on 5/27/2020 and 6/4/2020. The alarm history had "Williams Repair" <br /> written on the alarms but no documentation was written on the reports or work orders relating to the repair were <br /> found. <br /> REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> FA0003925 PR0232272 SCO01 04/22/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
