4/26/2021 <br /> 208: "The designated operator failed to document all alarms from the attached alarm history on the <br /> 5/27/20 and 6/24/2020 designated operator inspection reports" <br /> Prior to October 1, 2020 the Designated Operator inspection reports did not require the documentation <br /> of non LEAK detection alarms on the inspection reports. <br /> See attached Designated Operator inspection reports from 5/27/2020 and 6/24/2020. <br /> Designated Operator inspection report form Section VII.Alam Histo "Has each LEAK detection alarm <br /> since the previous inspection been responded to appropriately" <br /> The L3 diesel Annular space high and low alarms that were occurring were due to brine and sensor level <br /> adjustment issues that coincided with the site receiving diesel fuel deliveries, not leak alarms. <br /> Gavin Williams from "Williams Repair" had been working with Doug Smith on the issue. I wrote <br /> "Williams Repair" on the alarm history attached to the Designated Operator reports from 5/27/2020 <br /> and 6/24/2020 because"Williams Repair" had been addressing the ongoing adjustment issue. <br /> 208; "and failed to check that they were responded to appropriately" <br /> I did check on the issue. <br /> 5/27/2020: L3 diesel Annual space high liquid alarmed 6:49am, 6:53am, &7:10am. <br /> 1 <br /> 12/27/2020 AM. Doug Smith and Gavin Williams from Williams Repair had been emalling back and forth <br /> on the issue.05/27/2020 10:44am alarms cleared and stopped had occurring.See attached emails. <br /> 12/27/202012:55pm: I arrive on site to preform Designated Operator Inspection. Finding the earlier <br /> that day L3 alarms on the alarm history. I go into the CORP Yard office and discussed the 1-3 alarms with <br /> Doug Smith. Doug shows me the communications between he &Williams Repair and that they are <br /> handling the adjustment issues.The issue was on going due to the fact that the alarms would occur <br /> when the site received diesel fuel deliveries.They would have to wait for the next delivery to see if the <br /> issue occurred again or not.This site does not receive frequent diesel fuel deliveries. <br /> 6/24/2020 12:18pm: I arrive on site to preform Designated Operator inspection,finding the 1-3 alarms <br /> that occurred 6/4/202012:30 LOW liquid, 12:44 HIGH liquid, 12:58 LOW liquid on the alarm history I go <br /> into the CORP Yard office to speak with Doug Smith regarding the alarms. Doug informs me that <br /> Williams Repair had made liquid and brine liquid adjustments. Site received diesel fuel delivery with no <br /> L3 alarms and that they believe the issue has been resolved. <br /> 6/26/2020 Doug Smith and Williams Repair email back and forth saying the issue has not occurred after <br /> the diesel fuel delivery, see attached email. Williams Repair sends invoice.See attached invoice. <br /> 7/22/20201 arrive on site to preform Designated Operator inspection.
