The following is an itemized list of hazardous materials violations that have not been <br /> addressed for SPEEDEE OIL CHANGE & TUNE-UP-MTCA as of March 09, 2021. <br /> Open violations from September 25, 2018 inspection <br /> Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> The employee training records were not available for review at time of inspection. The business plan shall include <br /> provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years.
