The following is an itemized list of hazardous materials violations that have not been <br /> addressed for MIRACLE MILE MARKET as of March 09, 2021. <br /> Open violations from December 09, 2020 inspection <br /> Violation#3 - Failed to establish and implement a hazardous materials business plan. <br /> OBSERVATION At the time of inspection, it could not be demonstrated that a Hazardous Material Business Plan <br /> has been established and implemented. When employee Abdullah was asked where the hazardous material <br /> business plan was, he did not know what or where it was. When asked about their emergency response <br /> procedures to a spill, Abdullah did not know where the spill kit was located until after making a phone call. <br /> REGULATION: A Hazardous Materials Business Plan shall be established and implemented for emergency <br /> response to a release or threatened release of a hazardous material when storing hazardous materials at or above <br /> the thresholds quantities of 55 gallons for materials that are liquids, 500 pounds for solids, or 200 cubic feet for <br /> compressed gas, at standard temperature and pressure. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the required information, and submit for review by the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #5 -Failed to electronically submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Observation: The Hazardous Materials Inventory forms for gasoline contains obsolete hazard categories. <br /> REGULATION: As of March 1, 2018, US EPA requires the use of twenty-four new federal hazard categories. The <br /> previous five federal hazard categories are obsolete and shall be corrected. This information must be current and <br /> submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, correct the hazard categories, and resubmit the chemical inventory to the EHD for <br /> approval.
