The following is an itemized list of hazardous materials violations that have not been <br /> addressed for MIRACLE MILE MARKET as of March 09, 2021. <br /> Open violations from December 09, 2020 inspection <br /> Violation#11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION The facility was unable to demonstrate and provide documentation that appropriate personnel <br /> have received training in safety procedures in the event of a release or threatened release of a hazardous material. <br /> When employee Abdullah was asked how he would respond if there was a spill, he did not know where the spill kit <br /> was located and how to respond. <br /> REGULATION: The business plan shall include provisions for ensuring that appropriate personnel receive initial <br /> and annual refresher training. All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and <br /> procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#12 -Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATIONt Facility could not provide the Emergency Response Plan and the Chemical inventory upon <br /> request. <br /> REGULATION: The emergency response plans and procedures, the inventory of information required by this <br /> article, and the site map required by this section shall be readily available to personnel of the business or the unified <br /> program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 4 of 4 <br />
