The following is an itemized list of hazardous materials violations that have not been <br /> addressed for MIDAS AUTO SERVICE CENTER as of March 09, 2021. <br /> Open violations from January 31, 2019 inspection <br /> Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms for all materials were not complete. Please add the new federal hazard <br /> categories to the inventory sheet. Please change the obsolete hazard classes to say no. This information must be <br /> current and submitted as part of the business plan. Immediately log into the California Environmental Reporting <br /> System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6-Failed to complete and/or submit a site map with all required content. <br /> The site map was not complete as part of the business plan. The site map is missing a key or legend that defines E, <br /> F, HMS, HWS, ESD, R/R, H,W, +. Please add Central street,the parking lots, and the adjacent building, strom <br /> drains, utilities shut offs, and an evacuation staging area. A site map shall contain a north orientation, loading <br /> areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If <br /> a site map element is not applicable for your facility then list it on the map and label as"NA". The current emergency response and contingency plan <br /> is not readable. Please fill out the consolidated emergency response and contingency plan template. Please upload <br /> the the completed emergency response and contingency plan. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 3 <br />
