The following is an itemized list of hazardous materials violations that have not been addressed for MIDAS AUTO SERVICE CENTER as of March 09, 2021. Open violations from January 31, 2019 inspection Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. The Hazardous Materials Inventory forms for all materials were not complete. Please add the new federal hazard categories to the inventory sheet. Please change the obsolete hazard classes to say no. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#6-Failed to complete and/or submit a site map with all required content. The site map was not complete as part of the business plan. The site map is missing a key or legend that defines E, F, HMS, HWS, ESD, R/R, H,W, +. Please add Central street,the parking lots, and the adjacent building, strom drains, utilities shut offs, and an evacuation staging area. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA".
