Appeals <br />must ba filed with this Department within 1 O tlays of the action with an appeal fee of $353.00. The 10 -day appeal <br />period ends at 4:30 p. m. on July 23, 2021. If this date falls on a weekend or holiday, the appeal period will expire <br />on the next regular business day at 4:30 p. m. <br />EXPIRATION: This action requires you to comply with all Conditions of Approval by March 17, 2022, which is <br />11 years 6 months from the original effective date (September 6, 2011).
