The following is an itemized list of underground storage tank violations that have not <br /> been addressed for MIRACLE MILE MARKET as of March 09, 2021. <br /> Open violations from August 25, 2020 inspection <br /> Violation#208-Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days. <br /> OBSERVATION:The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -incorrect overfill test dates <br /> -Section VII was incomplete <br /> -service technician certification was expired on 02/23/20 for the 03/05/2020 report <br /> -04/09/2020 report was signed with a prior date of 01/06/2020 <br /> -some reports were not signed by the owner/operator <br /> -missing secondary test date <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> OBSERVATION:The designated operator inspections for the 04/09/2020 was performed more than 30 days after <br /> the previous inspection conducted on 03/05/2020. <br /> REGULATION GUIDANCE:The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 4 of 4 <br />
