On December 9 , 2020 , Vicky Vang - Lee of the San <br /> Joaquin County EHD performed a paperwork inspection . <br /> A full inspection report was issued on December 10 , 2020 identifying information to be submitted to <br /> bring this site into compliance . This information was required to be submitted by January 9 , 2021 . <br /> This information has not been received resulting in a non - compliant status for this facility . <br /> The outstanding violations are itemized on the following pages and may include violations from past <br /> inspections . Complete and submit a copy of the Return to Compliance Certification form to the EHD <br /> with a statement documenting the corrective actions that have been or will be taken of each violation , <br /> and any supporting paperwork . <br /> Note : All EHD staff time associated with failing to comply, including the issuance of this letter, <br /> is billed at the current hourly rate ( $ 152 ) .
