Date: <br /> Manufacturer training: <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> NOTES <br /> On site to verify one 6,000 gallon underground storage tank has been completely filled with controlled density fill slurry. <br /> A total of 41 cubic yards of slurry was used to fill the entire UST system. <br /> Approximately 1 to 2 cubic yards of sludge and 1,260 gallons of liquid was generated from triple rinsing the tank and piping. <br /> Ray Smith (terminal manager)states that the waste is pending a hazardous waste determination before deciding on an <br /> appropriate disposal of the waste. <br /> Piping in the sump was cut in the process of trying to enter the tank manway. The piping was placed into one 55 gallon metal <br /> drum, labeled as hazardous waste, and left on site. <br /> Please refer to the SR0082708 approval letter for additional action needed. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them <br /> sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
