S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Fe:CBUSO dba Woodbridge Winery 5950 E Woodbridge Rd, Acampo ptember 14, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete and is missing the following elements: <br /> -Loading Areas <br /> -Internal Roads <br /> -The site map only listed the location of the following materials:welding gases, motor oil, seamer oil, ingredients, <br /> argon,waste oil filters, ED-470, ammonia, nitrogen, Perma Treat, propane, DEF, sanitation chemicals, universal <br /> waste, diesel (missing one location),waste oil, glue,waste coolant, carbon dioxide, refrigeration oil, and lubo drive. <br /> The other chemicals listed in the inventory were not on the site map. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. <br /> The 2019 facility and inventory submittals were made on 1/24/2019. <br /> The 2019 plan submittal was made on 1/25/2019. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on September 21, 2021 and replaces the initial checklist. <br /> There were 3 violation(s) during the inspection. Complete and submit a copy of the Return to Compliance <br /> Certification form to the EHD with a statement documenting the corrective actions that have been or will be <br /> taken for each violation, and any supporting paperwork, by October 21, 2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign.
