Please provide the following additional information and address the following comments: <br /> 1. The Application for Underground Storage Tank Closure Permit is incomplete. The following information is <br /> missing: <br /> a. fire permit number <br /> b. county and phone number of laboratory <br /> 2. Item 7e list the disposal site for the rinsate as World Oil Recycling. This is not an appropriate disposal site. <br /> 3. Item 8b does not provide information for the hauler of the piping that will be removed. <br /> 4. Item 8c does not provide information for the disposal site for the piping that will be removed. <br /> 5. Item 9a does not provide boring information for piping <br /> 6. Item 11c states that soil boring waste are expected to be non-hazardous and hauled by Bellshire Environmental <br /> Services. All waste generated will be managed as hazardous until determined otherwise. <br /> 7. Item 14 was not <br /> answered. <br /> i <br />
