Environmental Health Department <br />AMENDED <br />10/28/2020 <br />Hazardous Materials Program Inspection Report <br />Date: <br />September 30, 2020 <br /> Facility Address: <br /> 1331 S HAM LN, LODI <br /> Facility Name: <br /> LODI MUNI SERVICE CENTER <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />The Hazardous Materials Inventory forms for the following material is not complete/accurate: <br />1. Propane – storage temperature is wrongly identified as “below ambient” vs. “ambient”. <br />2. Used lubricating oil – the annual waste amount element has not been completed. <br />3. Waste coolant - the annual waste amount has not been completed. <br />4. Electrolyte/sulfuric Acid - Electrolyte/sulfuric reported as "pounds" instead of "gallons". <br />This information must be current and submitted as part of the business plan. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br />submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />The site map submittal on CERS does not show/address: <br />1. Internal roads <br />2. Some codes used on the map are not defined <br />3. Gasoline and diesel tanks are not described with enough detail at the fueling island location. <br />A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as “NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />The emergency response(ER) procedures were lacking or inadequate since the emergency response procedures <br />uploaded into CERS is for the underground storage tank program. <br />A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />FA0003961 PR0520128 SC001 09/30/2020 <br />EHD 19-01 Rev. 9/16/2020 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
