Jasmin Sekhon, Merchandise Service Coordinator, stated that when there is a spill of a store item that is <br />difficult to clean-up, such as nail polish, a ticket is submitted on Ulta's online system regarding the spill and an <br />outside company is contracted to conduct the clean-up. Jasmin did not know the name of the company that is <br />called to perform these clean-ups, nor did Jasmin know how the company manages the waste generated from <br />these clean-ups. According to Jasmin, the company has performed a clean-up in the facility about three times in <br />the past two years. <br />Submit a statement and supporting documentation explaining which company was called to perform these <br />chemical spill clean-ups for the facility, and how the company managed the waste generated from these <br />clean-ups. <br />2. The inspection report was amended by adding Violation 403. <br />An inspection checklist was provided to the facility operator on the day of inspection.
