Violations 405 and 605 <br />have been closed. The following information is needed to close out the remaining open violations: <br /> <br />1. Violation 106 – Please submit proof of training for each employee that was re-trained by the General Manager. <br />All employees (managers, coordinators, stylists, associates, etc.) that handle or manage hazardous waste should <br />be re-trained, and proof of this re-training should be submitted. <br />2. Violation 102 – Your response to this violation is still under review.
