OBSERVATION: The designated operator failed to check and note the correct test dates for the overfill prevention equipment inspection on the designated operator inspection reports. Overfill prevention was incorrectly listed as 4/20/20. Testing was conducted on 4/23/19. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a minor violation. 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest opportunity. OBSERVATION: Test boots on six of the pipes in the UDCs were not pulled back and not positioned in a way that leaks will be detected at the earliest opportunity. On three pipes, hoses attached to the boots were looped upward. On two pipes, boots were not pulled back and the valves were sealed closed. On one pipe,jumper hoses were still attached on both ends. REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. CORRECTIVE ACTION: Ensure that any releases in the piping in the UDCs will be detected at the earliest opportunity. The boots were all pulled back by the technician at the time of inspection. This is a repeat violation, Class ll. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by May 13, 2021. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
