The permit has been <br />approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br />any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Provide written notification to ust ,si4ov.orq to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). <br />a. Overfill prevention verification: Measurement of drop tubes to verify 95% tank volume shut <br />off. Verification of flapper functionality. Ensure that all pertinent tank charts and <br />calculations are available for review at the time of inspection. <br />b. Verification that ball float(s), if installed, do not interfere with the installed flapper valve. <br />Please be aware that the facility cannot receive fuel in the 87, 91, and diesel tanks prior <br />to the EHD inspection and approval. <br />4. Any individual perf <br />orming the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be issued <br />to the installation company), or a Tank Tester's license issued to the individual by the State <br />Water Board <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service Technician <br />certificate issued by the International Code Council (ICC) <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced <br />5. Individuals installing underground storage tank system components must possess or work <br />under the direct and personal supervision of an individual physically present at the work site <br />who possesses all of the following: <br />1868 E.
