SAN, JOAQUIN Environmental Health Department <br /> COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ULTA BEAUTY#1314 5410 Pacific Ave, Stockton October 02, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Item # Remarks <br /> -A 6.75 oz bottle of Formula 10.0.6 So Totally Clean was identified as a Flammable Liquid And Solid on a <br /> hand-held scanner, which was used by Mercedes Alegre, Prestige Manager, to scan the item(IMG 8448-50). <br /> -A 4 oz bottle and a 1 oz bottle of Urban Decay All Nighter Makeup Setting Spray were labeled as flammable(IMG <br /> 8443). <br /> -A 5 oz aerosol can of KeraColor Color Me Clean Dry Shampoo, which Mercedes stated should have been placed <br /> in the Flammable Gas (FG) Hazardous Waste container(IMG 8451-3). <br /> 5. A container labeled as Oxidizer(OX) Hazardous Waste contained the following waste item (IMG 8471, 8473): <br /> -At least two 2 oz bottles of Redken SEQ Gloss were identified as Flammable Liquid And Solid(FL) on the scanner <br /> (IMG 8475-6). <br /> REGULATION GUIDANCE: All hazardous waste containers shall be accurately marked with the following <br /> information: the words "Hazardous Waste", name and address of generator, hazardous properties, physical state, <br /> composition (contents), accumulation start date. <br /> CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste accurately. If the <br /> container is emptied daily then the accumulation start date can be"emptied daily." Provide a corrective action <br /> statement and supporting documentation to the San Joaquin County Environmental Health Department(EHD) <br /> within 30 days. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> 1. A Damage/Destroy In Field Procedure Flow Chart was found in a hazardous waste manifest binder on a shelf <br /> above the hazardous waste storage area (IMG 8428). Provide a statement to the San Joaquin County <br /> Environmental Health Department explaining the process depicted in this flow chart, the types of items involved <br /> in this process, and if this process is still being used. <br /> An inspection checklist was provided to the facility operator on the day of inspection.
