Jasmin Sekhon, Merchandise Service Coordinator, stated that when there is a spill of a store item that is <br /> difficult to clean-up, such as nail polish, a ticket is submitted on Ulta's online system regarding the spill and an <br /> outside company is contracted to conduct the clean-up. Jasmin did not know the name of the company that is <br /> called to perform these clean-ups, nor did Jasmin know how the company manages the waste generated from <br /> these clean-ups. According to Jasmin, the company has performed a clean-up in the facility about three times in <br /> the past two years. <br /> Submit a statement and supporting documentation explaining which company was called to perform these <br /> chemical spill clean-ups for the facility, and how the company managed the waste generated from these <br /> clean-ups. <br /> 2.The inspection report was amended by adding Violation 403. <br /> An inspection checklist was provided to the facility operator on the day of inspection.
