Mann Singh was hired 4/23/2021, and was the only employee observed on site upon arrival <br /> for inspection. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br /> one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br /> site, and submit a copy of the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 307 HSC 25290.1(c3), 25290.2(c3) Failure to keep water out of the secondary containment system (post-Jul 2003). <br /> OBSERVATION: Water was found in the (87, 91, and Diesel) STP sump. <br /> REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the <br /> system by precipitation, infiltration, or surface runoff. <br /> CORRECTIVE ACTION: The service technician removed in total approximately half a gallon of liquid from the (87, <br /> 91, and Diesel) sump and returned the sensor to its proper location.
