Reviewed Cal OES spill <br />report #21-0434 and update, received via email at 5:38 am and 5:50 am, <br />respectively. Called and spoke to Tony Rocha, Lineage Logistics Facility <br />Maintenance Manager (209-239-2888, 209-564-0884, <br />arocha@lineagelogistics.com), at 3:30 am the anhydrous ammonia release alarm <br />went off and approximately 20 employees were evacuated from the building. At <br />4:45 am a Lineage Logistics ammonia refrigeration technician arrived on-site, and <br />at 5:05 am the technician identified the leak. The leak came from an evaporator <br />unit valve, which is believed to have loosened due to strong winds. The technician <br />tightened the valve and the leak stopped at -5:10-5:15 am. No more than 10 <br />pounds of anhydrous ammonia was released. The evacuation was lifted at 5:30 <br />am and all employees returned to work. No exposure, injuries, or hospitalizations <br />occurred. <br />PE 2546 FLORIDO <br />SC 907 <br />Notified Dr. Maggie Park, Interim Health Officer (mpark@sjcphs.org ), via email of PE 2546 FLORIDO <br />the release information obtained from Tony Rocha. CC'd Jasjit Kang, EHD SC 544 <br />Director, Muniappa Naidu, EHD Asst. Director, Melissa Nissim, CUPA Program <br />Coordinator, Elena Manzo and Haza Saeed, Ld Sr REHSs, in the email. <br />Generated complaint and began generating the Prop 65. Documented narrative. <br />Antonio Roach confirmed the information Elianna received. The release is from a <br />loose valve on the evaporative Coil. The valve was tightened and release was <br />stopped. <br />There was no ammonia Odor at the time of the inspection. <br />The valve was up high on the ceiling and could not been seen. Antonio showed <br />me a photo of the valve. They believe the valve was loose due to the high wind <br />coming from the docking doors. <br />Generated a generic inspection report and emailed to Antonio. Incident <br />investigation is in progress. Incident investigation report will be looked at during <br />the next CalARP routine inspection. <br />Received calls from Kimberley Blackwell, office assistant, Elena Manzo and Haza PE 2546 FLORIDO <br />Saeed, Ld Sr RENS, regarding the release. SC 007 <br />Elianna Florido received a compliant of an ammonia release today at this facility PE 2546 SAEED <br />and was given to me for follow-up. I looked up PE 2546 SAEED <br />the property owner mailing info and print the report to be mailed to the facility and SC 544 <br />property owner. <br />Fixed complaint city location and placed map coordinates in Envision Connect.
