SAN JOAQUIN
Environmental Health Department
COUNTY—
Complaint Record Daily Time and Activity Summary from 4/25/2015 to
2/26/2021for COPUQPLZW located at
730 SPRECKELS AVE
Activity Date Recorded by
01/27/2021
60 min
01/27/2021
90 min
01/27/2021
30 min
01/27/2021
120 min
6:30-7:30am: Notified by the San Joaquin General Hospital (468-6007) of an
ammonia release at 730 Spreckels Ave in Manteca. Reviewed Cal OES spill
report #21-0434 and update, received via email at 5:38 am and 5:50 am,
respectively. Called and spoke to Tony Rocha, Lineage Logistics Facility
Maintenance Manager (209-239-2888, 209-564-0884,
arocha@lineagelogistics.com), at 3:30 am the anhydrous ammonia release alarm
went off and approximately 20 employees were evacuated from the building. At
4:45 am a Lineage Logistics ammonia refrigeration technician arrived on-site, and
at 5:05 am the technician identified the leak. The leak came from an evaporator
unit valve, which is believed to have loosened due to strong winds. The technician
tightened the valve and the leak stopped at -5:10-5:15 am. No more than 10
pounds of anhydrous ammonia was released. The evacuation was lifted at 5:30
am and all employees returned to work. No exposure, injuries, or hospitalizations
occurred.
PE 2546 FLORIDO
SC 907
Notified Dr. Maggie Park, Interim Health Officer (mpark@sjcphs.org ), via email of PE 2546 FLORIDO
the release information obtained from Tony Rocha. CC'd Jasjit Kang, EHD SC 544
Director, Muniappa Naidu, EHD Asst. Director, Melissa Nissim, CUPA Program
Coordinator, Elena Manzo and Haza Saeed, Ld Sr REHSs, in the email.
Generated complaint and began generating the Prop 65. Documented narrative.
Antonio Roach confirmed the information Elianna received. The release is from a
loose valve on the evaporative Coil. The valve was tightened and release was
stopped.
There was no ammonia Odor at the time of the inspection.
The valve was up high on the ceiling and could not been seen. Antonio showed
me a photo of the valve. They believe the valve was loose due to the high wind
coming from the docking doors.
Generated a generic inspection report and emailed to Antonio. Incident
investigation is in progress. Incident investigation report will be looked at during
the next CalARP routine inspection.
Received calls from Kimberley Blackwell, office assistant, Elena Manzo and Haza PE 2546 FLORIDO
Saeed, Ld Sr RENS, regarding the release. SC 007
Elianna Florido received a compliant of an ammonia release today at this facility PE 2546 SAEED
and was given to me for follow-up. Lyida Baker with EHD and I responded and met SC 544
with the facility Plant Engineer, Antonio Rocha.
01/29/2021
18 min
02/01/2021
12 min
Updated the Prop 65 reports with my name and current information. I looked up PE 2546 SAEED
the property owner mailing info and print the report to be mailed to the facility and SC 544
property owner.
Fixed complaint city location and placed map coordinates in Envision Connect.
