Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COW7NCVWX Site Location: 3900 HOLLY DR <br />Activity Summary <br />Activity Date Recorded by <br />01/27/2021 Received spill report 21-0436 at 7:48 am. 2400 pounds of 19% liquid ammonia BAKER <br />released from a damaged pipe. Release occurred at City of Tracy's Wastewater <br />Treatment Plant at 3900 Holly Drive. Called Aoke Vaid at 209-831-6328 for details <br />regarding the release. A transfer line was damaged as a result of the storm. <br />Ammonia flowed onto the dirt, to a storm drain and then to the retention pond. <br />Heavy rains washed ammonia into retention pond. Facility believes dirt is not <br />impacted by release. Contents of retention pond will be put back into the waste <br />water treatment system. Discussed with Elena Manzo (Lead Sr. REHS), Haza <br />Saeed (Lead Sr. REHS) and Melissa Nissim (Program Coordinator). Called Aloke <br />Vaid with additional questions regarding spill. NRC was notified. No spill material <br />went into secondary containment. No evacuations or injuries. No ammonia can <br />be smelled right now. No ammonia alarms are going off currently. Aloke Vaid is <br />unsure if pond is lined but does not think so. Storm water and effluent that is not <br />approved for discharge typically go into the pond. <br />Left office to visit site. Arrived on site with Haza Saeed (Lead Sr. REHS). Met <br />with Aloke Vaid (Operations Superintendent) and Deborah DeWolf (Administrative <br />Technician). Spill is believed to have occurred at approximately 6:30 pm on <br />1/26/2021 when a ladder fell onto the pipe due to the high storm winds. Leak was <br />discovered when operator noticed ammonia smell in the area. Pipe where leak <br />occurred was located between the ammonia tank and the shed with the pump. <br />Broken pipe has been repaired. Ammonia flowed onto ground in a southwest <br />direction. Two puddles of clear liquid were observed approximately 15 feet from <br />the leaking pipe. No smell of ammonia was detected in the area. Haza Saeed <br />used an ammonia detector while on site. No ammonia was detected by the <br />equipment while on site. Puddles were approximately 5x5 feet in size. pH was <br />taken of both puddles. pH result was 7-8 for both puddles. The heavy rains <br />caused the ammonia to flow to the storm drain approximately 100 feet away and <br />east of the original leak. No ammonia detected in area of the storm drain. Storm <br />drain leads to a pump. The pump leads to the retention pond. The retention pond <br />is approximately ten million gallons in size. Retention pond did not appear to be <br />lined. No ammonia detected in the area of the retention pond. At least 25 <br />ducks/geese were observed on the retention pond. Incident investigation by <br />responsible party has already been started. Per, Aloke Vaid, water in retention <br />pond will be tested prior to discharge. SJC EHD requested responsible party to <br />notify Regional Water Board of release. <br />Returned to office. Generated complaint, Emergency Response Record and site <br />map. Wrote Notice to Abate. Emailed Notice to Abate to Deborah DeWolf & <br />Aloke Vaid at deborah.dewolf@cityoftracy.org and aloke.vaid@cityoftracy.org. <br />Completed prop 65 and put on Elena Manzo's desk for review. <br />Completed photo log. Verified GIS in EC. <br />01/28/2021 Called Regional Water Board 5 at 916-464-3291 regarding spill. Given email for BAKER <br />Howard Hold at howard.hold@waterboards.ca.gov. Emailed Howard Hold about <br />complaint and Water Board requirements. Called and left a message for Howard <br />Hold at 916-464-4679. <br />5105.rpt Page 2 of 3
