Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />April 08, 2021 <br /> Facility Address: <br /> 15237 N Thornton Rd, Lodi <br /> Facility Name: <br /> FLYING J TRAVEL PLAZA #617 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for "Used Gasoline and Diesel Filters" and <br />"Contaminated Absorbents" were not complete. The Chemical Hazard Classification section for these materials <br />state "no" for all questions indicating there are no hazards associated for these materials. <br />REGULATION GUIDANCE: The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. <br />- Carbon Dioxide is not indicated on the map <br />- Spill control equipment is not indicated on the map <br />- First aid kits and fire extinguishers in the store are not indicated on the map <br />- Loading areas are not listed on the map <br />- Internal roads are not listed on the map <br />- Listed Used Gasoline and Diesel filters on the inventory are not listed on the map <br />- Listed Contaminated Absorbents on the inventory are not listed on the map. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment.
