Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />April 13, 2021 <br /> Facility Address: <br /> 3555 W HAMMER LN, STOCKTON <br /> Facility Name: <br /> QUIK STOP MARKET #132 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. <br />- loading areas are not listed on the map <br />- a fire extinguisher in the fuel canopy was not listed on the map <br />- the pump shut off switch is located outside of the store but is shown as inside the store on the map <br />- there is a fire extinguisher located in the storage room behind the cashiers that is not listed on the map <br />- there is a fire extinguisher listed on the map next to the store entrance but it was not observed on site <br />- define acronyms (HMH, FL, HMS, INT, RR) on the map <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. <br />- 2020 HMBP submittal was submitted late on 1/20/2020 <br />- 2019 HMBP submittal was submitted late on 2/12/2019 <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Ensure submittals are annually submitted and complete by January 15 of each year. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were 2 violation(s) during the <br />inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br />statement documenting the corrective actions that have been or will be taken for each violation, and any <br />supporting paperwork, by 5/13/2021. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152).
