The following is an itemized list of hazardous materials violations that have not been <br />addressed for Michael's New York Style Pizza, Inc as of May 17, 2021. <br />Open violations from April 12, 2021 inspection <br />❑ This violation was corrected <br />❑ Supporting documents included <br />❑ This violation will be corrected by (date): <br />Describe actions taken or will be taken to correct violation: <br />Violation #8 -Failed to establish and electronically submit a reasonable or an adequate training program. <br />OBSERVATIOI�t The Employee Training Plan has not been annually certified/submitted as a part of the business <br />plan. The last submittal was completed on 5/17/2017. The Employee Training plan has not been submitted for <br />2018, 2019, 2020 and 2021. <br />REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br />the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br />States Code. An annual electronic submittal satisfies these requirements. The annual business plan shall be <br />submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br />previous year. <br />CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, review and submit/certify the <br />Curren 'nformation. <br />This violation was corrected ❑This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #9 -Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATIO�t The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. The last submittal was completed <br />on 5/17/2017. The business plan has not been submitted for 2018, 2019, 2020 and 2021. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br />information, and submit to the EHD for approval. <br />Page 3 of 5 <br />
