SANJOAQUIN <br />—COUNTY — <br />(7reorneSs grows here <br />Environmental Health Department <br />Time In: 2:31 pm <br />Time Out: 3:00 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: OMELETTE HOUSE Date: 03/15/2021 <br />Address: 3455 E CHEROKEE RD, STOCKTON 95205 <br />Owner/Operator: DIAB, FAROUK Telephone: (209) 931-1717 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00053514 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT ALLEGES THE TABLES AND FLOORS ARE REALLY DIRTY AND THERE WERE ROACHES <br />CRAWLING FROM THE FLOOR TO THE TABLE <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION -- TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />I went to the facility and spoke to an employee who was not aware of compliant. <br />I also observed one table with dining and employees not wearing masks. <br />I explained to employee that dining in is not allowed at this time and that masks are to be worn. <br />Employee said that the manager was not available. <br />I inspected the seating area and observed black debris accumulation at the juncture of booths and flooring. <br />Clean and mop more frequently. <br />During my investigation the owner arrived. <br />Owner was not aware of complaint. <br />Per owner pest control is in place and services facility once per month. <br />I did not "Observe roaches crawling from the floor to the table.." during investigation. <br />FA0002019 C00053514 SC004 03/15/2021 <br />EH D 16-23 Rev. 01/07/2021 <br />Page 1 of 2 Complaint Inspection Report <br />1868 E.
