ACTIVITY HAZARD ANALYSIS 01 DPT <br />Activity: Direct-Push Analyzed By/Date: <br /> <br />Reviewed By: <br />Page 2 of 8 <br /> <br />Principal Steps <br /> <br />Potential Hazards <br /> <br />Recommended Controls <br /> <br />Drilling operations; set Uwork area and movi. <br /> <br />Electrocutions, explosions, disastrous <br />events, etc. <br /> <br />Obtain and examine copies of all pertinent station <br />drawings prior to performing the task. Locate and mark <br />out the existing underground utilities. Obtain <br />Underground Service Alert clearance prior to work. <br />Inspect the area of drilling activity for overhead <br />obstructions. Personnel will contact the service <br />facilities engineer before working near utilities. Ensure <br />that weight of rig is evenly distributed on ground and is <br />not so heavy as to damage any underground lines that <br />may be near the surface. <br /> <br />rig into position. (cont.) <br /> <br />Rig could contact overhead lines, <br />including power lines, if it is transported <br />with rig raised, causing electric shock. <br />Vehicle may move if not properly set up <br /> <br />Never move the rig when the mast is extended. <br /> <br />Use spotter to properly position vehicle. Set brakes <br />and place wheel chocks under front wheels of mobile <br />rig. Extend stabilizer jacks, and ensure that footing is <br />sound. Vehicle must be level to the vertical and <br />horizontal planes. <br /> <br />When raising rig, rig may not install <br />properly due to the condition of rig and <br />connecting cables. <br />When raising rig, mast could come in <br />contact with, or be in close proximity to, <br />overhead power lines causing <br />electrocution of workers. <br /> <br />Inspect all components of rig to determine condition. <br />Make all repairs before raising rig. <br /> <br />The mast and other equipment must be at least <br />15 feet from any overhead utility lines. Verify the <br />voltage of any overhead power lines. If any lines <br />are above 50 kilovolt (kV), the clearance distance <br />must be greater. Refer to Engineer Manual 385- <br />1-1, Section 11, Table 11-3 for clearance <br />required for voltages above 50kV. <br />When any part of the rig or equipment is in motion, <br />worker will stand far enough away from the moving <br />parts so that the worker is not pinned between the <br />moving parts. Workers will not manually guide any <br />moving part of the rig when it is raised up. Workers will <br />not work under the rig or under the truck. If work must <br />be done under the rig or truck, the drill crew supervisor <br />will contact the Site Health and Safety Specialist to <br />ascertain a safe method for lock-out of the equipment <br />to ensure that adequate blocking is installed. <br /> <br />Worker may become pinned <br />between rig and other truck <br />components, or worker could be <br />pinned under truck rig if rig must be <br />serviced from under the truck. <br />
