Site Safety and Health Plan <br />Stockton Field Maintenance Shop #24 <br />8020 Airport Way, Stockton, CA <br />1.0 INTRODUCTION <br /> <br />1.1 PURPOSE <br />Star Resources, Inc. (Star), views the implementation of a Site Safety and Health Plan (SSHP) <br />as a critical management element necessary to the success of a soil gas investigation at <br />Stockton Field Maintenance Shop (FMS) #24, Stockton, CA. The SSHP describes the activities <br />to be performed and identifies procedures to be followed to minimize the potential for personnel <br />exposure to contaminants known to be present at the site. All Star employees and <br />subcontractors who perform field work during the project will be required to read this SSHP <br />before performing any field activities. <br />This SSHP is part of the periodic review and modification process of the overall health and <br />safety program for the site, which occurs throughout the duration of the project to ensure <br />flexibility and adaptability as changes occur and new situations develop. Modifications to this <br />SSHP are reflective of the Stipulated Notice and Order, as well as Title 27, California Code of <br />Regulations, Sections 20919.5(f) and 20921(d), and have been approved by the Star Project <br />Manager (PM), the Site Safety and Health Officer (SSHO) and the Corporate Safety and Health <br />Manager. <br /> <br />1.2 SCOPE OF THE SSHP <br />The SSHP addresses installing, sampling, and destroying four temporary soil gas probes at <br />Stockton FMS #24. The SSHP also addresses: <br />Potential contaminants of concern and their toxicity, symptoms of exposure, and <br />properties; <br />Responsibilities of key site safety and health personnel; <br />Work practices and Standard Operating Procedures (SOPs); <br />Hazard identification and assessment, including chemical, physical, biological, and <br />radiation hazards; <br />Establishment of work zones; <br />Level of personal protective equipment (PPE) and required clothing in each zone during <br />each task; <br />Exposure monitoring/air sampling procedures; <br />Heat/cold stress monitoring; <br />Entry and exit routes; <br />Decontamination procedures; <br />Response procedures for accidents and emergencies; <br />Emergency contacts and phone numbers; <br />Medical surveillance; <br />Training requirements for workers and on-site training; and <br />Recordkeeping requirements. <br />Star Resources Corp 6
