Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: C00053560 Site Location: 1000 E ROTH RD <br />Activity Summary <br />Activity Date Recorded by <br />03/17/2021 10:48 am Received spill report from Elena Manzo 'Lead Senior REHSI. GAITAN <br />Approximately 100 gallons of diesel was released on to asphalt from Crane. Site <br />located at 1000 E. Roth Rd. French Camp Rd. Contacted Michael Algots, HazMat <br />Supervisor for Union Pacific. Stated that Patriot Environmental was contacted to <br />clean up spill. Stated Site Glass on Crane malfunctioned and caused spill, Michael <br />provided me photo of initial release. Contacted Elena, discussed complaint, <br />based on discussion I left to observe spill and clean up. <br />Met with Crane Mechanic, Arnie Depietro, Stated that spill occurred around 10:30 <br />am. Used nearby spill kit to contain the diesel. Stated that the Crane clamp <br />malfunctioned causing diesel release, the leak was stopped. I observed no water <br />ways or soil impacted by diesel. <br />Crane number: 90410 serial number 1521 was the malfunctioned crane. <br />Contacted Michael, updated him on diesel spill, asked for ETA for Patriot <br />Environmental, stated that they were 15 minutes out. Updated Elena on complaint. <br />Generated photo log. Contacted Elena, updated her on spill, based on discussion I <br />would be generating an OIR. <br />Officer Rodriguez, state and wildlife. Contacted me asking about more information <br />regarding the spill. I let him know that no waterways or soil were impacted from <br />the 100 gallon diesel release. <br />Generated complaint inspection report, emailed to Michael Algots. Generated <br />confidential list. <br />Brandon Eck, supervisor for Patriot environmental arrived with clean up crew. <br />They poured absorbent on asphalt impacted with diesel. Patriot crew let absorbent <br />sit for approximately 20 minutes, crew then swept up absorbent contaminated with <br />diesel and placed into 55-gallon containers. A total of three 55-gallon containers <br />were generated from clean up. I observed no diesel sheen or odor, clean up was <br />adequate. <br />Brandon stated all containers were going to remain on site in the hazardous waste <br />accumulation area. Waste will be managed accordingly pending analytical. <br />Notified Michael Algots, UPRR let him know that I deemed the clean up to be <br />adequate. Contacted Elena updated her on complaint.
