The 6/19/20, 7/15/20, 12/16/20, 2/2/21, 2/25/21, and 3/23/21 inspection reports were not signed and/or dated by <br /> the owner/operator. <br /> 2. The 9/29/20 inspection report did not include the owner/operator description of follow up actions. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection.
