The administrative action is to ensure compliance with <br />applicable statutes and assess penalties for violations. A copy of the inspection report and a <br />non-compliance letter identifying outstanding violations, are enclosed. <br />EHD has found that the Facility is/was in violation of the following regulations: <br />1. HSC 25505(a)(2) Failed to complete and/or submit a site map with all required <br />content <br />2. HSC 25505(a)(4)Failed to provide or document initial and/or refresher training to <br />appropriate personnel <br />3. HSC 25505(c) Failed to have the business plan readily available to facility or EHD <br />personnel <br />All penalties for these violations will be calculated pursuant to the appropriate California law. <br />Some applicable penalties may accrue on a daily basis unless and until the conditions are <br />abated. <br />1868 E.
