The following is an itemized list of hazardous materials violations that have not been <br />addressed for CVIN - SAN JOAQUIN COE as of October 07, 2019. <br />Open violations from September 27, 2018 inspection <br />Violation #6 -Failed to complete and/or submit a site map with all required content. <br />The map did not show storm and sewer drains, and evacuation staging areas. A site map shall contain a north <br />orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br />emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br />response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br />"NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers,calepa.ca,gov/, <br />upload the correct or updated information, and submit to the EHD for approval. <br />❑ This violation was corrected ❑This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #11 -Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Employee training records were not available at the time of inspection. The business plan shall include provisions <br />for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br />documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br />provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br />❑ This violation was corrected <br />❑ <br />❑ This violation will be corrected by (date): <br />Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #12 -Failed to have the business plan readily available to facility or EHD personnel. <br />Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br />response plans and procedures, the inventory of information required by this article, and the site map required by <br />this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br />for emergency response or training pursuant to this section.
