A site map shall contain a north <br />orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br />emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br />response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br />"NA". Immediately log into the California Environmental Reporting System (CERS) at hftp://cers.Galepa.ca.gov/, <br />upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br />11 <br />HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Employee training records were not available at the time of inspection. The business plan shall include provisions <br />for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br />documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br />provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br />This is a Class II violation. <br />12 <br />HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br />Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br />response plans and procedures, the inventory of information required by this article, and the site map required by <br />this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br />for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br />plan readily available for review. <br />This is a minor violation. <br />Overall Inspection Comments. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the completed inspection report. <br />Facility does not have loading areas, internal roads, emergency response equipment (fire extinguisher and spill <br />control kept on company vehicle) and emergency shutoffs and these elements were not addressed on the site <br />map. Please address this by either writing "NA" for not applicable or a statement that says the element is not on <br />site. Please correct this by the next submittal. <br />The electrolyte is below reportable quantity and may be removed for the chemical inventory. <br />Starting September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152).
