A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> H&M- BW#98 2501 JACKSON AVE, ESCALON July 20, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> The following were not observed on site during the inspection: <br /> -7/29/2019 Monitoring System Certification results <br /> -4/11/2019 Overfill Prevention Equipment results <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 301 HSC 25292.1(a)Failure to operate UST system to prevent unauthorized releases including leaks,spills and overfills. <br /> OBSERVATION: The fuel filters under both dispensers(1/2 and 3/4)are not located to allow drips and leaks to drain <br /> back into the UDC. <br /> REGULATION GUIDANCE: The underground storage tank system shall be operated to prevent unauthorized <br /> releases. <br /> CORRECTIVE ACTION: Immediately provide approved secondary containment for the filters under permit and <br /> inspection by the EHD. <br /> This is a Class II violation. <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br /> OBSERVATION: The float and chain in dispenser 1/2 for 87 grade failed to stop the flow of product at the dispenser <br /> when tested. <br /> REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. <br /> CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 8/19/2021. <br /> FA0003910 PR0231488 SCO01 07/20/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
