SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: T6633 <br /> ility Address: Date: <br /> PACIFIC AVE CHEVRON PACIFIC AVE, STOCKTON May 03, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: Eighteen 15 pound Blue Rhino containers of propane were found on site that have not been <br /> reported to CERS. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The follow items were missing or incorrectly located: <br /> -The Emergency Pump Shut-Off is located inside the cashier area on the site map. The Emergency Pump Shut-Off <br /> was observed outside at the northeast corner of the store <br /> -A Carbon Dioxide tank was observed outside the store on the south side and was not included on the site map <br /> -Storm drains were not marked as not applicable on the site map <br /> -Internal roads were not included on the site map <br /> -Loading areas were not included on the site map <br /> -Access&exit points were not designated on the site map <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class Il. <br /> FA0003834 PR0520539 SCO01 05/03/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 4 Hazardous Materials Program OIR <br /> 1868 E.
