The Primary Emergency <br />Contact 24 hour phone number was wrongly reported as 209-957-2000 vs 209-747-4155. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />6 <br />HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The site map does not show/address the following required elements: <br />1. Loading areas. <br />2. Internal roads. <br />3. Access and exit points from the street. <br />4. Emergency response equipment (fire extinguishers and spill control) <br />5. A legend to define codes/abbreviations used on the map has not been provided (FE, STMD, SEWD) <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br />If a site map element is not applicable for your facility then list it on the map and label as "NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class 11. <br />9 <br />HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information was not reviewed and resubmitted in the California Environmental <br />Reporting System (CERS) annually by the due date, January 15.
