SAN J OA U I Environmental Health Department <br /> COUNTY- AMENDED <br /> 5/4/2021 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: F7732 <br /> lity Address: Date: <br /> CARLS JR#7484 PACIFIC AVE, STOCKTON May 04, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information was not reviewed and resubmitted in the California Environmental <br /> Reporting System (CERS) annually by the due date, January 15. The business plan was not submitted for the <br /> reporting year 2020 and was not submitted until 2/2/21 for the reporting year 2021. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Conducted in-office Hazardous Materials Business Plans (HMBP) inspection. An inspection report was provided <br /> to the Environmental Contact on May 4, 2021. Later the same day, the report was amended by the EHD. This <br /> amended report replaces the initial report. <br /> The report was amended to add violation 5 for failure to update the obsolete federal categories for carbon <br /> dioxide. <br /> Food inspector, Maribel Flohrschutz, performed an on-site inspection and verified the presence of the CO2 tank <br /> on January 14, 2021. <br /> There were 4 violations during the in-office inspection. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 3, 2021. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
