SAN dDAQU <br />C01-1 NTY <br />Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />MANTECA WELL #29 <br />614 EL PORTAL AVE, MANTECA <br />May 03, 2021 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />5 <br />HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: Approximately 100 gallons of sodium hypochlorite and 400 gallons of diesel were found on site <br />that have not been reported to CERS. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br />approval. <br />This is a Class II violation. <br />6 <br />HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map was not completed and submitted as part of the business plan. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br />If a site map element is not applicable for your facility then list it on the map and label as "NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />7 <br />HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />OBSERVATION: The emergency response(ER) procedures were not submitted. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />FA0026555 PR0546873 SCO01 05/03/2021 <br />EHD 19-01 Rev. 9/16/2020 Page 3 of 6 Hazardous Materials Program OIR <br />1868 E.
