The following is an itemized list of hazardous materials violations that have not been <br /> addressed for ACME SAW & INDUSTRIAL SUPPLY as of September 30, 2021. <br /> Open violations from May 12, 2021 inspection <br /> Violation#5-Failed to electronically submit hazardous materials inventory for all reportable materials on <br /> site. <br /> OBSERVATION: The Hazardous Materials Inventory forms for the saw grinding fluid were not complete. <br /> 1.According to George Earl, employee,the one large surface/knife grinder holds approximately 150 gallons of saw <br /> grinding fluid diluted with water, and the three smaller surface grinders each hold approximately 50 gallons, 50 <br /> gallons, and 25 gallons. However,the hazardous materials inventory states the largest container holding saw <br /> grinding fluid is 5 gallons and the max daily amount is 60 gallons. <br /> 2. The quantity of the largest container of CLC Carb-O-Grind 33L is not reported on the inventory forms.According <br /> to the hazardous materials inventory,the max daily amount of CLC Carb-O-Grind 33L is 80 gallons.According to <br /> George Earl,the grinding equipment in the mechanical area holds 80 gallons of CLC Carb-O-Grind 33L. <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6-Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The locations of the CLC Carb-O-Grind 33L and the saw grinding fluid <br /> are missing from the site map. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 2 <br />
