The following is an itemized list of hazardous materials violations that have not been addressed for ACME SAW & INDUSTRIAL SUPPLY as of September 30, 2021. Open violations from May 12, 2021 inspection Violation#5-Failed to electronically submit hazardous materials inventory for all reportable materials on site. OBSERVATION: The Hazardous Materials Inventory forms for the saw grinding fluid were not complete. 1.According to George Earl, employee,the one large surface/knife grinder holds approximately 150 gallons of saw grinding fluid diluted with water, and the three smaller surface grinders each hold approximately 50 gallons, 50 gallons, and 25 gallons. However,the hazardous materials inventory states the largest container holding saw grinding fluid is 5 gallons and the max daily amount is 60 gallons. 2. The quantity of the largest container of CLC Carb-O-Grind 33L is not reported on the inventory forms.According to the hazardous materials inventory,the max daily amount of CLC Carb-O-Grind 33L is 80 gallons.According to George Earl,the grinding equipment in the mechanical area holds 80 gallons of CLC Carb-O-Grind 33L. REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part of the business plan. OBSERVATION: The site map is incomplete. The locations of the CLC Carb-O-Grind 33L and the saw grinding fluid are missing from the site map. REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA".
