The following site map elements are incomplete: <br />-Evacuation staging area is not included on the site map <br />-Loading areas are not included on the site map <br />-A storm drain was observed south west of the store and was not included on the site map <br />-Three fire extinguishers were observed on the fuel island and were not included on the site map <br />-Fuel tank emergency shut off was observed inside the store back room and was not included on the site map <br />-The fire extinguisher inside the store on the site map was observed in a different location on the south west corner <br />of the store <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. The Hazardous Materials Inventory <br />and Emergency Response & Training Plan submittals for 2021 were submitted late on 2/6/2021. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: This was corrected on 2/6/2021. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection report was provided to the facility operator on the day of inspection. The report was amended and <br />replaces the initial report. Violation #9 was changed to corrected on site. <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were two violations during the <br />inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br />statement documenting the corrective actions that have been or will be taken for each violation, and any <br />supporting paperwork, by June 24, 2021. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152).
