F) Automatic alarms and shut-off devices. <br /> The permittee shall install automatic alarms and shut-off <br /> devices which will immediately notify the operators of a <br /> malfunction -in .the event that ,the injection pressure:. at. ,, :- <br /> the <br /> t. , -the well head exceeds the maximum injection pressure <br /> authorized by this permit. The automatic alarms shall <br /> also notify the operators of a significant change in the, <br /> annular fluid volume and pressure in accordance with Part <br /> II, Section B, item 6, of this permit. <br /> The equipment shall be installed and operating prior to <br /> commencing injection of waste fluid into the injection <br /> well . <br /> G) Plugging and abandonment plan <br /> The permittee shall submit a complete plugging and aban- . <br /> donment plan in accordance with Part I, Section G, of <br /> this permit. The plugging and abandonment plan shall <br /> provide all cost estimates for supplies, use of equip- <br /> ment, and service's necessary to properly plug and abandon <br /> the well. The information submitted shall include a com- <br /> pleted EPA Form 7520-14, Plugging and Abandonment Plan. <br /> H) Injection fluid analysis <br /> The permittee shall submit a complete characterization of <br /> the waste stream in accordance with 40 CFR <br /> §146. 12 (c) (2) (ii) and §146. 14 (a) (7)4(iii) . At least 30 <br /> days prior to collecting the representative sample, the <br /> permittee shall submit a plan to EPA which indicates the <br /> constituents that will be tested for in the waste 'stream <br /> and the test methods that will be used. This plan must <br /> be approved by EPA before the permittee shall collect the <br /> samples for analysis. <br /> 23 <br />
